The Beneficial use of the PIER approach to transform a deteriorating patient's treatment and health

The PIER approach (Prevention, Identification, Escalation, Response) is a beneficial way of ensuring that treatments are effectively planned for patients suffering with deteriorating health. Implementing this approach is highly beneficial as it not only eases the process of managing complex states of health deterioration but it can also help reduce the length of hospital stays, ensuring patients can return sooner to the comfort of their homes as well as easing the management of resources at organisations.

According to NHS England, PIER stands for:

  • prevention: planning ahead of any episode of deterioration to stop what is preventable, considering indicators of risk and patient choice
  • identification: tools and methods to identify when deterioration is occurring in a standardised way
  • escalation: timely escalation of care when deterioration has been identified using standardised communication tools
  • response: timely, appropriate and effective response to escalation of the deteriorating patient/person.
